When do you treat chronic hypertension in pregnancy?
Based on findings from the Chronic Hypertension and Pregnancy (CHAP) Study, ACOG now recommends initiating or titrating antihypertensive therapy when blood pressure reaches ≥140/90 mm Hg, replacing the older threshold of 160/110 mm Hg.
This recommendation reflects data showing improved maternal and perinatal outcomes when chronic hypertension is treated using the lower threshold, without increasing the risk of fetal growth restriction.
